Set Up Direct Deposit




Most employers have Direct Deposit forms to allow you to directly deposit your paycheck into your account(s). 

If your employer does not have a form, you can request direct deposit of your paycheck by completing the following information and then providing it to your employer.

After completing this form, you will need to print out the resulting page and take it to your payroll department, along with a voided check from your account.  If you cannot print the page, please call 1-800-580-0322 Ext. 2440 for assistance.
